Sir: I read with interest the letter to the editor authored by Briken and colleagues.1 Although I certainly appreciate the points raised in their case report, it is important to consider the following issues when evaluating and treating patients afflicted with paraphilic disorders. (1) As documented in the orchiectomy and pharmacotherapy literature, paraphilic patients remain at risk for sexual offense recidivism irrespective of the biological treatment they are receiving.​
