SAMe 200 mg plus Lactobacillus plantarum HEAL9 1 × 10^9 CFU once daily improved self-rated depressive symptoms more than placebo in adults with subthreshold or mild-to-moderate depressive symptoms over 6 weeks. The primary endpoint, change in Zung Self-Rating Depression Scale (Z-SDS) total score at week 6, favored the combination (P = .0165), and the Z-SDS core depression subdomain also improved more than placebo (P = .0247). An earlier between-group benefit was already present at week 2 for Z-SDS total score (P = .0330).
-
The study found an early signal by 2 weeks. Compared with placebo, the combination produced a greater reduction at treatment week 2 in Z-SDS total score (P = .0330), the Z-SDS cognitive subdomain (P = .0133), the Z-SDS anxiety subdomain (P = .0459), and the Zung Self-Rating Anxiety Scale total score (P = .0345). The authors concluded that the combination showed fast and clinically relevant effects after 2 weeks of intake.
-
The anxiety findings were mixed but favored the combination early in treatment. At week 2, SAMe plus Lactobacillus plantarum HEAL9 improved the Z-SDS anxiety subdomain (P = .0459) and the Zung Self-Rating Anxiety Scale total score (P = .0345) more than placebo. However, the between-group difference in Zung Self-Rating Anxiety Scale total score was not significant at the end of treatment at week 6 (P = .3418).
-
No. In the exploratory analysis within the active-treatment group, baseline Z-SDS total score did not influence the degree of symptom reduction at the end of treatment, with Spearman ρ = 0.01 and P = .9684. Within the enrolled range of subthreshold to mild-to-moderate symptoms, the observed improvement did not appear to depend on initial symptom severity.
-
Not clearly. The analysis of change from baseline to the end of treatment on the other questionnaires showed no statistically significant between-group differences. A greater reduction in the Birmingham IBS symptom questionnaire total score was seen with the combination, but it did not reach statistical significance (P = .0854).
-
The combination was well tolerated over 6 weeks, and no treatment-related adverse events were reported. Adverse events occurred in 1 subject receiving placebo and 2 subjects receiving the combination, but these events were judged unrelated to study products. In the safety population of 89 subjects, no unblinding occurred, and compliance of at least 80% was recorded for 95.6% of treated patients.

The studied regimen was 1 oral tablet daily for 42 consecutive days. Each active tablet contained SAMe 200 mg plus Lactobacillus plantarum HEAL9 at 1 × 10^9 CFU, taken with a small amount of water, preferably without food.
-
The trial enrolled adults aged 18 to 60 years with recurrent mild-to-moderate depressive disorders according to ICD-10/F33 criteria and a baseline Zung Self-Rating Depression Scale raw score of 41 to 55. Patients were recruited from 10 Italian general practitioners, and 90 patients were randomized: 46 to SAMe plus Lactobacillus plantarum HEAL9 and 44 to placebo.
-
This was a 6-week randomized, double-blind, placebo-controlled trial, which supports a controlled comparison between the active combination and placebo. The primary outcome was the change from baseline to week 6 in Zung Self-Rating Depression Scale total score, analyzed with analysis of covariance. The results are most directly applicable to short-term treatment of adults with subthreshold or mild-to-moderate depressive symptoms in a primary care setting, using patient-reported outcome measures.
-
The main limitations were the relatively small sample size, the short 6-week treatment period, and the use of the Zung Self-Rating Depression Scale without a professional psychiatric evaluation. The authors also noted that no long-term conclusions can be drawn and that this is the only study so far evaluating this specific combination, with no reported dose-response data or data on use alongside antidepressant medications.
