Eibert R. Heerdink, PharmD, PhD, is affiliated with the Division of Pharmacoepidemiology and Clinical Pharmacology at Utrecht University and the Department of Clinical Pharmacy at Utrecht University Medical Center, as well as the Research Group Innovation of Pharmaceutical Care at the University of Applied Sciences Utrecht, all in The Netherlands.
